Roles & Responsibilities

• Provide compassionate hands-on patient care in a rehab setting to support recovery
• Administer medications and treatments accurately and document results promptly
• Collaborate with nurses, therapists and physicians to create and follow care plans
• Monitor patient status, report changes and escalate concerns to the care team
• Support activities of daily living and encourage patient independence during rehab
• Maintain accurate, timely charting and assist with discharge planning when needed

Qualifications

• Current LPN license in good standing (state licensure required)
• Basic Life Support (BLS) certification required or obtained upon hire
• Experience or comfort working in long-term care or rehabilitation preferred
• Current immunizations and ability to pass background and health screenings
• Strong communication skills and a team-player attitude
• Ability to provide reliable clinical documentation and follow facility policies

Education

• Graduate of an accredited Practical Nursing program; high school diploma or equivalent required

Knowledge & Skills

• Fundamentals of nursing care including vitals, medication administration and wound care
• Rehab-focused skills such as mobility assistance and restorative nursing techniques
• Clear and compassionate communication with patients, families and interdisciplinary teams
• Accurate electronic and paper charting practices
• Time management and the ability to prioritize patient safety
• Infection control practices and adherence to facility protocols
