Roles & Responsibilities

• Provide compassionate, hands-on care to residents — help with bathing, dressing, feeding, and toileting
• Assist with mobility and safe transfers to keep residents comfortable and safe
• Take and record vital signs and report changes in condition to the nursing team
• Document care accurately and timely in the resident chart
• Support activity participation and social engagement to promote resident well-being
• Follow infection control procedures and maintain a clean, safe environment
• Collaborate with nurses, therapists, and support staff to deliver person-centered care

Qualifications

• Active Licensed Nursing Assistant (LNA) credential — T-LNA experience a plus
• CPR/BLS certification required or able to obtain quickly
• Current immunizations and ability to pass required health screenings
• Background check and drug screen clearance as required by facility
• Comfortable assisting with transfers and ADLs; good physical endurance
• Customer-service mindset with strong communication skills
• Previous long-term care or skilled nursing experience preferred

Education

• High school diploma or GED and completion of an accredited LNA training program

Knowledge & Skills

• Knowledge of ADLs, transfers, and basic nursing care
• Strong infection control and safety practices
• Ability to take and document vital signs accurately
• Comfortable using basic electronic charting or EHR systems
• Clear, compassionate communication with residents and team members
• Time management and teamwork in a fast-paced care setting
