Roles & Responsibilities

• Provide compassionate direct patient care including medication administration and assessments
• Perform focused nursing assessments and document changes in patient condition clearly and promptly
• Collaborate with the interdisciplinary team to ensure coordinated, high-quality care
• Implement care plans and assist with ADLs while promoting patient independence and dignity
• Maintain accurate, timely charting in the facility’s electronic health record
• Follow facility policies and state regulations, including maintaining required licensure and DHS account
• Communicate effectively with patients, families, and staff to support positive outcomes

Qualifications

• Active Licensed Practical Nurse (LPN) license
• Active Delaware business license required to staff in DE
• Must have an account with the Delaware Department of Health and Social Services (DHS)
• Current BLS/CPR certification (or ability to obtain prior to start)
• Up-to-date immunizations and ability to pass background and drug screening
• Strong communication, organization, and teamwork skills

Education

• LPN diploma or vocational nursing certificate and active LPN licensure

Knowledge & Skills

• Medication administration and basic clinical nursing skills
• Patient assessment and documentation best practices
• Comfortable using electronic health records and charting systems
• Infection control, safety, and standard precautions
• Time management and the ability to prioritize in a fast-paced environment
• Professionalism and the ability to work collaboratively with clinical teams
