Roles & Responsibilities

• Deliver compassionate, high-quality nursing care to residents at the Hopkins Center
• Administer medications and treatments accurately and according to orders
• Perform focused patient assessments and monitor for changes in condition
• Document care clearly and timely in the electronic medical record
• Collaborate with nurses, therapists, and physicians to coordinate patient-centered care
• Provide patient and family education with patience and empathy
• Support admissions, transfers, and safe discharge processes

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• Completion of an accredited practical nursing program
• Current BLS/CPR certification or willingness to obtain prior to start
• Comfort working in a long-term care / skilled nursing environment
• Reliable, professional, and able to communicate clearly with patients and team members

Education

• Graduate of an accredited practical nursing (LPN) program

Knowledge & Skills

• Medication administration and basic IV/shot competency where applicable
• Wound care, vital signs monitoring, and basic patient assessment skills
• Familiarity with charting and electronic medical records
• Infection prevention practices and safe care techniques
• Strong interpersonal skills and ability to work collaboratively in a fast-paced setting
