Roles & Responsibilities
- Provide medical direction and clinical supervision for participant care across the PACE program
- Lead and oversee the Quality Assurance Performance Improvement (QAPI) program to improve outcomes
- Develop and maintain medical policies, procedures, standards and clinical protocols
- Supervise and mentor Primary Care Providers (PCPs) and Advanced Practice Providers (APPs)
- Conduct history & physicals, event-based and periodic reassessments, and develop individualized treatment plans
- Collaborate actively with the interdisciplinary team and participate in team and family conferences
- Educate participants and families about health maintenance, prognosis and chronic disease management
- Participate in provider on-call rotation and provide clinical guidance to staff in the community
Qualifications
- Board Certified in Family Medicine or Internal Medicine
- Fellowship training or Certificate of Added Qualifications (CAQ) in Geriatric Medicine
- Minimum of 1 year experience working with frail or elderly populations
- Valid driver's license, proof of insurance and reliable transportation
- Willingness to travel regionally as needed
- Interest in permanent placement (position is not open to Locums)
- Not accepting visa candidates (US work authorization required)
Education
- Board Certification in Family Medicine or Internal Medicine
- Fellowship training or Certificate of Added Qualifications in Geriatric Medicine
Knowledge & Skills
- Clinical care and chronic disease management for older adults
- Strong physical assessment and history-taking skills
- Leadership and clinical supervision of medical teams
- Quality assurance and performance improvement (QAPI) experience
- Interdisciplinary team collaboration and effective communication
- Proficiency with Microsoft Office (Word, Excel, PowerPoint) and basic email/calendaring systems
- Organizational, customer service, flexibility and problem-solving abilities
