Roles & Responsibilities
- Provide compassionate direct nursing care to residents in a skilled nursing and rehab setting
- Administer medications and treatments accurately, following facility protocols
- Monitor and document vital signs, changes in condition, and patient responses
- Support rehabilitation goals by assisting with ADLs and restorative nursing activities
- Collaborate with interdisciplinary team members to create and update care plans
- Perform wound care, dressing changes, and basic clinical procedures as needed
- Educate residents and families about care plans and aftercare instructions
- Maintain clear, timely documentation in the medical record and communicate concerns to leadership
Qualifications
- Current, active Licensed Practical Nurse (LPN) license (must meet state requirements for practice in New Mexico)
- BLS/CPR certification required; ACLS or other certifications a plus
- Previous experience in long-term care, nursing rehab, or skilled nursing preferred
- Ability to pass background check, drug screen, and required immunizations
- Comfort working an early morning start and extended day shift schedule
- Strong communication skills and a teamwork mindset
Education
- Diploma or certificate in Practical Nursing (LPN)
Knowledge & Skills
- Medication administration and safe documentation practices
- Basic nursing assessments and recognition of changes in condition
- Experience with wound care and restorative nursing techniques
- Familiarity with electronic medical records (EMR) and charting
- Infection control practices and resident safety procedures
- Time management and prioritization during busy shifts
- Compassionate patient and family education skills
- Ability to collaborate effectively with interdisciplinary care teams
