Roles & Responsibilities

• Conduct thorough history and physical examinations for new participants and reassessments
• Develop individualized medical treatment plans and oversee clinical care across settings
• Provide medical direction and supervision for PCPs and Advanced Practice Providers
• Lead and oversee the Quality Assurance Performance Improvement (QAPI) program
• Participate actively in intake and assessment decisions for PACE applicants
• Serve as a key member of the interdisciplinary team—educate and advise on diagnoses, function, and treatment options
• Collaborate with nursing on treatment programs and coordinate participant care
• Participate in provider on-call rotation and manage event-based clinical needs
• Contribute to development and maintenance of medical policies, procedures, standards, and protocols
• Communicate prognosis and care plans with participants and families to support informed decision-making

Qualifications

• MD or DO degree (Physician-level qualification expected)
• Active, unrestricted medical license in Massachusetts (or ability to obtain)
• Clinical experience with frail older adults and chronic disease management
• Experience in clinical leadership, supervision of PCPs/APPs, or medical director responsibilities
• Strong skills in history & physical, diagnostic evaluation, and medical documentation
• Ability to participate in provider on-call rotation and handle event-based clinical needs
• Excellent communication skills for working with participants, families, and an interdisciplinary team

Education

• MD or DO (Physician)
• Board certification in Family Medicine, Internal Medicine, or Geriatrics preferred

Knowledge & Skills

• Clinical assessment and physical exam expertise for older adults
• Chronic disease management and care planning
• Quality Assurance Performance Improvement (QAPI) principles
• Team leadership and interdisciplinary collaboration
• Care coordination across settings including community and nursing home care
• Medical policies, procedures, and standards development
