Roles & Responsibilities
- Provide skilled therapeutic care to residents as a Physical Therapist Assistant
- Implement individualized treatment plans under PT direction to maximize patient function
- Document resident progress and treatment accurately and timely
- Collaborate with nurses, therapists, and the interdisciplinary team to coordinate care
- Educate residents and families on home programs and safety strategies
- Use clinical judgment to ensure resident safety and prevent falls
- Participate in care planning and team meetings to advocate for patient needs
Qualifications
- Licensed Physical Therapist Assistant in Ohio or eligible to obtain licensure prior to start
- Graduate of an accredited PTA program; new grads are welcome and encouraged to apply
- Current BLS/CPR certification or willingness to obtain prior to start
- Comfort working in a Skilled Nursing Facility setting
- Ability to start the 13-week assignment beginning 2025-12-15 as requested
- Clear background check and compliance with facility immunization policies
Education
- Associate degree from an accredited Physical Therapist Assistant program (or equivalent). New graduates accepted.
Knowledge & Skills
- Hands-on experience or training in therapeutic exercise and modalities
- Skilled documentation in electronic medical records and care plans
- Strong communication and teamwork skills for SNF-based care
- Ability to perform safe patient transfers and mobility training
- Knowledge of rehab protocols for geriatric and post-acute populations
