Roles & Responsibilities

• Evaluate patients thoroughly to identify functional deficits and create individualized treatment plans
• Implement evidence-based occupational therapy interventions focused on ADLs, transfers, mobility, and cognition
• Document progress clearly and timely to support care continuity and regulatory requirements
• Collaborate with nursing and rehab team members to coordinate effective care plans
• Educate patients and caregivers to maximize independence and safe discharge outcomes
• Adjust treatment plans responsively based on patient progress and goals

Qualifications

• Active Occupational Therapy license in Tennessee or ability to obtain state licensure prior to start
• Graduation from an accredited Occupational Therapy program (BS/BSOT/OTD) — new grads encouraged to apply
• Comfort working in a skilled nursing facility (SNF) environment
• Strong documentation skills and familiarity with therapy billing and compliance
• Professional communication skills and a collaborative mindset

Education

• Degree in Occupational Therapy (BS/BSOT/OTD or equivalent) required; new graduates accepted

Knowledge & Skills

• Skilled in ADL assessment and treatment planning
• Experience with mobility training, transfers, and adaptive equipment
• Familiar with SNF workflows, interdisciplinary team collaboration, and discharge planning
• Competent in clinical documentation and therapeutic progress measurement
• Ability to prioritize caseloads and manage time effectively in a busy facility
