Roles & Responsibilities
- Evaluate patients thoroughly to identify functional deficits and create individualized treatment plans
- Implement evidence-based occupational therapy interventions focused on ADLs, transfers, mobility, and cognition
- Document progress clearly and timely to support care continuity and regulatory requirements
- Collaborate with nursing and rehab team members to coordinate effective care plans
- Educate patients and caregivers to maximize independence and safe discharge outcomes
- Adjust treatment plans responsively based on patient progress and goals
Qualifications
- Active Occupational Therapy license in Tennessee or ability to obtain state licensure prior to start
- Graduation from an accredited Occupational Therapy program (BS/BSOT/OTD) — new grads encouraged to apply
- Comfort working in a skilled nursing facility (SNF) environment
- Strong documentation skills and familiarity with therapy billing and compliance
- Professional communication skills and a collaborative mindset
Education
- Degree in Occupational Therapy (BS/BSOT/OTD or equivalent) required; new graduates accepted
Knowledge & Skills
- Skilled in ADL assessment and treatment planning
- Experience with mobility training, transfers, and adaptive equipment
- Familiar with SNF workflows, interdisciplinary team collaboration, and discharge planning
- Competent in clinical documentation and therapeutic progress measurement
- Ability to prioritize caseloads and manage time effectively in a busy facility
