Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents in a long-term care/skilled nursing setting
• Administer medications and treatments accurately and document in the electronic medical record
• Perform thorough assessments and communicate changes in condition to the care team
• Collaborate with nurses, therapists, and providers to deliver individualized care plans
• Support residents with activities of daily living while promoting independence and dignity
• Monitor and manage wound care, vital signs, and safety precautions with attention to detail
• Participate in shift handoffs and maintain clear, timely documentation

Qualifications

• Current LPN license in Rhode Island (required)
• Recent experience as an LPN in long-term care, skilled nursing, or similar setting preferred
• Basic Life Support (BLS/CPR) certification required or willing to obtain
• Strong clinical assessment and medication administration skills
• Reliable, professional, and able to consistently cover Sunday and Tuesday doubles plus Friday
• Excellent communication and teamwork skills with a resident-centered approach

Education

• LPN/Vocational Nursing diploma or certificate
• High school diploma or GED

Knowledge & Skills

• Solid knowledge of medication administration, wound care, and infection control
• Comfortable with electronic medical records and timely documentation
• Skilled in resident assessment, vital sign monitoring, and care planning
• Ability to prioritize tasks in a fast-paced environment and handle multiple responsibilities
• Strong interpersonal skills for working with residents, families, and interdisciplinary teams
