Roles & Responsibilities
- Provide direct patient care with compassion and clinical accuracy
- Administer medications and document treatments according to facility protocols
- Monitor patient status, report changes promptly, and collaborate with the care team
- Perform wound care, catheter care, and other skilled nursing tasks as assigned
- Support residents with activities of daily living while promoting independence
- Communicate clearly with patients, families, and interdisciplinary staff
- Maintain accurate, timely charting and comply with regulatory requirements
Qualifications
- Current LPN license in Massachusetts required or compact equivalent where applicable
- Recent LPN experience in skilled nursing, rehab, or long-term care preferred
- Basic Life Support (BLS) certification required or must obtain upon hire
- Valid ID and ability to complete facility onboarding and background checks
- Strong communication skills and a patient-centered approach
Education
- Graduate of an accredited Licensed Practical Nurse (LPN) program
Knowledge & Skills
- Solid clinical assessment and medication administration skills
- Knowledge of nursing documentation practices and electronic medical records
- Familiarity with long-term care and rehab nursing workflows
- Ability to work collaboratively with nurses, therapists, and support staff
- Time management and prioritization in a fast-paced clinical setting
