Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately and document per facility policy
• Perform thorough assessments and communicate changes in condition to the care team
• Collaborate with CNAs, RNs, therapists, and interdisciplinary staff to support resident goals
• Maintain accurate electronic and paper records, ensuring compliance with regulatory standards
• Support restorative and daily living activities to maximize resident independence
• Participate in shift handoffs and contribute to a positive safety-focused culture

Qualifications

• Current Licensed Practical Nurse (LPN) license — Massachusetts license preferred
• Basic Life Support (BLS) certification required or willingness to obtain promptly
• Previous experience in long-term care, rehab, or skilled nursing preferred
• Strong clinical assessment and medication administration skills
• Excellent communication, teamwork, and documentation abilities
• Ability to pass background and health screenings required by the facility

Education

• Vocational nursing diploma or equivalent completion of an accredited LPN program
• High school diploma or GED

Knowledge & Skills

• Medication administration and IV basics for LPN scope
• Wound care, dressing changes, and skin integrity monitoring
• Familiarity with long-term care and post-acute rehab nursing workflows
• Electronic medical record charting and accurate documentation
• Infection control practices and adherence to facility policies
• Prioritization and time management during 12-hour shifts
