Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents in a skilled nursing setting
- Administer medications and treatments accurately and document per facility policy
- Perform thorough assessments and communicate changes in condition to the care team
- Collaborate with CNAs, RNs, therapists, and interdisciplinary staff to support resident goals
- Maintain accurate electronic and paper records, ensuring compliance with regulatory standards
- Support restorative and daily living activities to maximize resident independence
- Participate in shift handoffs and contribute to a positive safety-focused culture
Qualifications
- Current Licensed Practical Nurse (LPN) license — Massachusetts license preferred
- Basic Life Support (BLS) certification required or willingness to obtain promptly
- Previous experience in long-term care, rehab, or skilled nursing preferred
- Strong clinical assessment and medication administration skills
- Excellent communication, teamwork, and documentation abilities
- Ability to pass background and health screenings required by the facility
Education
- Vocational nursing diploma or equivalent completion of an accredited LPN program
- High school diploma or GED
Knowledge & Skills
- Medication administration and IV basics for LPN scope
- Wound care, dressing changes, and skin integrity monitoring
- Familiarity with long-term care and post-acute rehab nursing workflows
- Electronic medical record charting and accurate documentation
- Infection control practices and adherence to facility policies
- Prioritization and time management during 12-hour shifts
