Roles & Responsibilities

• Provide compassionate, hands-on care to residents, ensuring their comfort and dignity
• Administer medications and treatments accurately and on schedule
• Perform thorough patient assessments and monitor changes in condition
• Document care and observations clearly in the medical record
• Collaborate with the interdisciplinary team to create and follow individualized care plans
• Assist with activities of daily living and support resident mobility and safety
• Communicate effectively with families to keep them informed and involved

Qualifications

• Current LPN license in Massachusetts or compact/state-appropriate LPN licensure
• CPR/BLS certification (or willingness to obtain prior to start)
• Recent experience in long-term care, rehab, or skilled nursing settings preferred
• Clear background check and ability to pass any required health screenings
• Reliable, professional demeanor with strong communication and teamwork skills

Education

• Practical Nursing diploma or equivalent successful completion of an accredited LPN program

Knowledge & Skills

• Skilled in medication administration and basic wound care
• Strong patient assessment and charting abilities
• Comfortable supporting activities of daily living and mobility assistance
• Able to work collaboratively within a multidisciplinary care team
• Familiarity with electronic charting and standard clinical documentation practices
