Roles & Responsibilities
- Provide compassionate, hands-on care to residents, ensuring their comfort and dignity
- Administer medications and treatments accurately and on schedule
- Perform thorough patient assessments and monitor changes in condition
- Document care and observations clearly in the medical record
- Collaborate with the interdisciplinary team to create and follow individualized care plans
- Assist with activities of daily living and support resident mobility and safety
- Communicate effectively with families to keep them informed and involved
Qualifications
- Current LPN license in Massachusetts or compact/state-appropriate LPN licensure
- CPR/BLS certification (or willingness to obtain prior to start)
- Recent experience in long-term care, rehab, or skilled nursing settings preferred
- Clear background check and ability to pass any required health screenings
- Reliable, professional demeanor with strong communication and teamwork skills
Education
- Practical Nursing diploma or equivalent successful completion of an accredited LPN program
Knowledge & Skills
- Skilled in medication administration and basic wound care
- Strong patient assessment and charting abilities
- Comfortable supporting activities of daily living and mobility assistance
- Able to work collaboratively within a multidisciplinary care team
- Familiarity with electronic charting and standard clinical documentation practices
