Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments safely and accurately on schedule
• Perform routine assessments including vital signs and skin integrity checks
• Collaborate with nurses, therapists and support staff to implement individualized care plans
• Document care clearly and timely to support continuity and regulatory compliance
• Support resident mobility and rehabilitation goals through attentive care and encouragement
• Promote a safe environment by following infection control practices and facility policies

Qualifications

• Active Licensed Practical Nurse (LPN) license in Massachusetts
• Current BLS/CPR certification (or ability to obtain prior to start)
• Clinical experience in long-term care, skilled nursing or rehab settings preferred
• Ability to pass background checks, drug screening and required health screenings
• Strong communication skills and a team-oriented mindset

Education

• Graduate of an accredited LPN program (diploma or certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Resident assessment and recognition of changes in condition
• Basic wound care and dressing change familiarity
• Care planning, charting and timely documentation
• Effective communication with residents, families and interdisciplinary teams
• Time management and prioritization in a fast-paced clinical setting
