Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled nursing and rehab setting
- Administer medications and treatments safely and accurately on schedule
- Perform routine assessments including vital signs and skin integrity checks
- Collaborate with nurses, therapists and support staff to implement individualized care plans
- Document care clearly and timely to support continuity and regulatory compliance
- Support resident mobility and rehabilitation goals through attentive care and encouragement
- Promote a safe environment by following infection control practices and facility policies
Qualifications
- Active Licensed Practical Nurse (LPN) license in Massachusetts
- Current BLS/CPR certification (or ability to obtain prior to start)
- Clinical experience in long-term care, skilled nursing or rehab settings preferred
- Ability to pass background checks, drug screening and required health screenings
- Strong communication skills and a team-oriented mindset
Education
- Graduate of an accredited LPN program (diploma or certificate)
Knowledge & Skills
- Medication administration and documentation best practices
- Resident assessment and recognition of changes in condition
- Basic wound care and dressing change familiarity
- Care planning, charting and timely documentation
- Effective communication with residents, families and interdisciplinary teams
- Time management and prioritization in a fast-paced clinical setting
