Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as an LPN, keeping dignity and comfort front and center
• Administer medications and treatments accurately and document thoroughly in the chart
• Perform routine assessments and report changes in condition to the care team promptly
• Collaborate with RNs, providers, and interdisciplinary team members to execute individualized care plans
• Assist with admissions, transfers and discharges to ensure smooth transitions
• Maintain infection control standards and a safe, tidy environment for residents
• Engage residents and families with clear communication and supportive guidance

Qualifications

• Current LPN license (eligible to practice in Rhode Island preferred)
• Completion of an accredited practical nursing program
• Basic Life Support (BLS/CPR) certification required or willingness to obtain
• Comfortable working evenings and every-other-weekend rotations
• Previous long-term care or skilled nursing experience preferred
• Strong clinical assessment, medication administration and documentation skills

Education

• Completion of an accredited LPN/Practical Nursing program

Knowledge & Skills

• Solid knowledge of medication administration and resident safety
• Skilled in basic wound care, ADL assistance and resident hygiene needs
• Proficient with electronic health record documentation and charting
• Good clinical judgment, time management and teamwork under shift pressures
• Friendly communication skills for interacting with residents and families
