Roles & Responsibilities

• Provide compassionate direct patient care as an LPN, delivering daily nursing interventions with skill and heart
• Administer medications and treatments accurately and document timely in the patient record
• Monitor patient status, recognize changes, and escalate concerns to the care team quickly
• Assist with wound care, ADLs, and restorative nursing to help patients maintain dignity and independence
• Collaborate with RNs, therapists, and interdisciplinary team members to create and follow person-centered care plans
• Maintain accurate, timely charting and communicate effectively with patients, families, and staff
• Support a safe, sanitary environment by following infection control and facility policies

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing (required)
• Basic Life Support (BLS) certification from an accredited provider or willingness to obtain
• Ability to pass background check and drug screening as required by facility
• Current immunizations and TB screening per facility policy
• Previous long-term care or med-surg experience preferred but new travel LPNs with solid fundamentals are encouraged to apply
• Professional, compassionate communication skills and a teamwork mindset

Education

• Graduate of an accredited Practical Nursing program (LPN diploma/certificate)
• High school diploma or GED required

Knowledge & Skills

• Strong medication administration and documentation skills
• Clinical assessment skills for monitoring vitals and recognizing patient changes
• Familiarity with electronic medical records and accurate charting
• Basic wound care and ADL assistance competence
• Infection control, safety practices, and patient-centered care approaches
• Good time management and ability to prioritize in a fast-paced environment
