Roles & Responsibilities

• Assess patients and develop individualized occupational therapy plans that address real daily needs
• Provide skilled OT services across hospital, nursing home and outpatient settings to maximize recovery
• Treat roughly 6 to 8 patients per day and manage follow-up visits as required for inpatient care
• Collaborate with COTAs, rehab aides and the interdisciplinary team to deliver coordinated care
• Document care accurately and promptly in PointClickCare to keep patient records clear and compliant
• Communicate with patients and families about goals, progress and safe discharge planning

Qualifications

• Current Occupational Therapist licensure appropriate for practice in Minnesota (professional license required)
• BLS (Basic Life Support) certification
• Minimum of 2 years of OT experience (inpatient/acute or rehab experience preferred)
• Experience with charting/documentation and familiarity with PointClickCare is a strong plus
• Comfort working with patients who have debilitating diseases and across multiple care settings
• Ability to collaborate with COTAs and other rehab staff and to complete a 12-hour orientation

Education

• Degree in Occupational Therapy and current state licensure (OTR) as required

Knowledge & Skills

• Skilled in patient assessment and developing measurable OT treatment plans
• Strong documentation and charting practices
• Experience in acute inpatient, acute rehab, skilled nursing and outpatient care
• Familiarity with PointClickCare electronic charting
• Ability to work effectively with interdisciplinary teams and supervise/coordinate with COTAs
