Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a rehabilitation setting
- Administer medications and treatments safely and accurately
- Assess, document, and communicate patient status and changes to the interdisciplinary team
- Support individualized care plans that promote recovery and functional independence
- Collaborate with physicians, therapists, and family members to deliver coordinated care
- Assist with admissions, transfers, and discharges while maintaining high standards of documentation
- Advocate for patient comfort, dignity, and safety at every shift
Qualifications
- Active Licensed Practical Nurse (LPN) license for Maryland (or active RN license if applying as an RN)
- Current BLS/CPR certification (American Heart Association or equivalent) recommended
- State licensure or ability to obtain Maryland licensure in a timely manner
- Previous experience in rehabilitation, skilled nursing, or similar long-term care preferred
- Strong communication skills and the ability to work collaboratively on a multidisciplinary team
Education
- Graduate of an accredited LPN program
- High school diploma or GED required
Knowledge & Skills
- Solid knowledge of basic nursing procedures, medication administration, and vital sign monitoring
- Comfortable with documentation and charting; familiarity with electronic medical records is a plus
- Good clinical judgment, prioritization, and time-management skills
- Ability to provide patient-centered care with empathy and professionalism
- Familiarity with rehab-focused care plans and restorative nursing principles
