Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a rehabilitation setting
• Administer medications and treatments safely and accurately
• Assess, document, and communicate patient status and changes to the interdisciplinary team
• Support individualized care plans that promote recovery and functional independence
• Collaborate with physicians, therapists, and family members to deliver coordinated care
• Assist with admissions, transfers, and discharges while maintaining high standards of documentation
• Advocate for patient comfort, dignity, and safety at every shift

Qualifications

• Active Licensed Practical Nurse (LPN) license for Maryland (or active RN license if applying as an RN)
• Current BLS/CPR certification (American Heart Association or equivalent) recommended
• State licensure or ability to obtain Maryland licensure in a timely manner
• Previous experience in rehabilitation, skilled nursing, or similar long-term care preferred
• Strong communication skills and the ability to work collaboratively on a multidisciplinary team

Education

• Graduate of an accredited LPN program
• High school diploma or GED required

Knowledge & Skills

• Solid knowledge of basic nursing procedures, medication administration, and vital sign monitoring
• Comfortable with documentation and charting; familiarity with electronic medical records is a plus
• Good clinical judgment, prioritization, and time-management skills
• Ability to provide patient-centered care with empathy and professionalism
• Familiarity with rehab-focused care plans and restorative nursing principles
