Roles & Responsibilities

• Evaluate patients to identify functional needs and develop individualized occupational therapy plans
• Deliver evidence-based interventions to improve activities of daily living and independence
• Document patient assessments, treatment plans, and progress in the medical record accurately and timely
• Collaborate with nursing, physical therapy, speech therapy and the interdisciplinary team to coordinate care
• Educate patients and families on adaptive strategies, home safety and use of assistive devices
• Participate in discharge planning and recommend community resources to support transitions
• Adjust treatment approaches based on objective outcomes and patient-centered goals

Qualifications

• Current Occupational Therapist license in Maine or ability to obtain state licensure
• Graduation from an accredited Occupational Therapy program (Master's or OTD preferred)
• NBCOT certification preferred
• Basic Life Support (BLS) certification preferred or willing to obtain
• Ability to pass background checks and meet facility immunization requirements
• Strong documentation skills and comfort working with electronic medical records

Education

• Master's degree or Doctorate (OTD) in Occupational Therapy from an accredited program

Knowledge & Skills

• Proficient in OT evaluation, treatment planning and progress measurement
• Skilled in ADL/IADL training, adaptive equipment recommendations and functional mobility
• Experience with therapeutic modalities relevant to skilled nursing/rehab settings
• Clear, patient-centered communication and ability to educate families and staff
• Team-oriented mindset with strong clinical reasoning and time management
