Roles & Responsibilities
- Evaluate and assess patient mobility, function, and rehabilitation needs with thorough, evidence-based evaluations
- Develop individualized treatment plans to restore function and improve quality of life
- Deliver hands-on therapy and therapeutic exercises to help patients meet their goals
- Document progress clearly and timely in the medical record to ensure continuity of care
- Collaborate with nurses, OT, physicians, and support staff to coordinate comprehensive care
- Provide patient and caregiver education to promote safe discharge plans and home programs
- Participate in facility orientation and team meetings to support a strong clinical culture
Qualifications
- Licensed Physical Therapist (PT) in California (or eligible to obtain license prior to start)
- Current BLS/CPR certification (or willing to obtain)
- Experience in acute/medical rehabilitation settings preferred
- Strong clinical assessment and manual therapy skills
- Ability to pass facility background checks and required health screenings
- Professional communication skills and a patient-centered approach
Education
- Graduation from an accredited Physical Therapy program (DPT preferred)
Knowledge & Skills
- Skilled in patient assessment and goal-oriented treatment planning
- Proficient with therapeutic modalities and rehabilitative exercise programs
- Clear documentation and charting practices
- Ability to work collaboratively within an interdisciplinary team
- Time management and caseload prioritization
- Knowledge of safety, infection control, and compliance standards
