Roles & Responsibilities
- Provide patient-centered occupational therapy evaluations and develop individualized treatment plans
- Implement evidence-based OT interventions to improve activities of daily living and functional independence
- Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomes
- Document assessments, progress notes, and discharge plans accurately and on time
- Educate patients and families on adaptive strategies, home modifications, and assistive devices
- Monitor and adjust treatment plans based on patient progress and clinical judgment
Qualifications
- Current and unrestricted California Occupational Therapy license (required)
- Certified Occupational Therapist with NBCOT certification preferred
- Minimum clinical experience working with adult or geriatric populations preferred
- Active BLS/CPR certification or willingness to obtain upon hire
- Strong communication skills and experience working in interdisciplinary teams
- Ability to pass facility onboarding requirements and background checks
Education
- Graduation from an accredited Occupational Therapy program (OTR or equivalent)
Knowledge & Skills
- Skilled in OT evaluations, treatment planning, and ADL training
- Experience with therapeutic modalities, adaptive equipment, and home safety assessments
- Familiarity with documentation standards and electronic medical records
- Ability to prioritize caseloads and manage time effectively
- Strong interpersonal skills for patient and family education
- Problem-solving mindset with attention to clinical detail and outcomes
