Roles & Responsibilities

• Provide patient-centered physical therapy evaluations and develop individualized treatment plans
• Deliver skilled interventions including therapeutic exercise, manual therapy, and mobility training
• Monitor patient progress and adjust plans to maximize functional outcomes
• Collaborate with nurses, physicians and interdisciplinary team members to coordinate care
• Document assessments, treatments and outcomes accurately in the facility EMR
• Educate patients and families on home programs and safe mobility strategies
• Support discharge planning to ensure safe transitions and continuity of care

Qualifications

• Current Massachusetts Physical Therapy license or ability to obtain state licensure promptly
• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy considered
• Basic Life Support (BLS) certification required
• Comfortable working in a post-acute/contract therapy setting (experience preferred)
• Strong documentation skills and familiarity with facility EMR systems
• Ability to pass background checks and meet facility onboarding requirements

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy accepted

Knowledge & Skills

• Skilled in patient assessment, treatment planning and therapeutic exercise
• Proficient in gait training, balance training and mobility assistance
• Knowledge of manual therapy techniques and assistive device training
• Strong clinical reasoning and outcome-driven care approach
• Excellent communication and interdisciplinary collaboration skills
• Efficient documentation and time-management in a fast-paced setting
