Roles & Responsibilities
- Provide patient-centered physical therapy evaluations and develop individualized treatment plans
- Deliver skilled interventions including therapeutic exercise, manual therapy, and mobility training
- Monitor patient progress and adjust plans to maximize functional outcomes
- Collaborate with nurses, physicians and interdisciplinary team members to coordinate care
- Document assessments, treatments and outcomes accurately in the facility EMR
- Educate patients and families on home programs and safe mobility strategies
- Support discharge planning to ensure safe transitions and continuity of care
Qualifications
- Current Massachusetts Physical Therapy license or ability to obtain state licensure promptly
- Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy considered
- Basic Life Support (BLS) certification required
- Comfortable working in a post-acute/contract therapy setting (experience preferred)
- Strong documentation skills and familiarity with facility EMR systems
- Ability to pass background checks and meet facility onboarding requirements
Education
- Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy accepted
Knowledge & Skills
- Skilled in patient assessment, treatment planning and therapeutic exercise
- Proficient in gait training, balance training and mobility assistance
- Knowledge of manual therapy techniques and assistive device training
- Strong clinical reasoning and outcome-driven care approach
- Excellent communication and interdisciplinary collaboration skills
- Efficient documentation and time-management in a fast-paced setting
