Location: Pleasant Hill, CA
Contract Type: Perm
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (30-minute meal), weekend rotation, day shift
Hours per Week: 35
Candidate Pay Rate: $48 per hour

Roles & Responsibilities
- Provide skilled physical therapy interventions under the direction of a licensed PT to improve patient mobility, strength, and independence
- Assist with gait training, transfers, therapeutic exercises, and balance programs tailored to each patient
- Document patient progress accurately and timely in the facility’s clinical records
- Collaborate with nursing, rehab team members, and families to develop and execute individualized care plans
- Educate patients and caregivers on safe mobility techniques and home exercise programs
- Participate in team meetings and contribute to quality improvement initiatives that enhance patient outcomes
Qualifications
- Active Physical Therapist Assistant license in California (required)
- CPR/BLS certification (current) or willingness to obtain upon hire
- Experience in post-acute, skilled nursing, or rehabilitation settings preferred
- Strong patient-handling skills and ability to work with medically complex populations
- Excellent communication and teamwork skills with a compassionate approach to care
Education
- Associate Degree in Physical Therapist Assistant or equivalent accredited program
Knowledge & Skills
- Familiarity with therapeutic exercise, gait training, transfers, and assistive devices
- Skilled in clinical documentation and basic EMR navigation
- Knowledge of infection control and safe patient handling techniques
- Ability to assess patient response and modify interventions accordingly
- Strong interpersonal skills and ability to collaborate across disciplines
