Roles & Responsibilities
- Deliver patient-centered physical therapy evaluations and individualized treatment plans
- Use evidence-based interventions to improve mobility, function, and independence
- Collaborate with nursing, OT, SLP, and care teams to coordinate holistic care
- Document skilled care accurately and timely to support clinical outcomes
- Educate patients and families on home exercise programs and safe transfers
- Participate in discharge planning to ensure smooth transitions and continuity
Qualifications
- Current California Physical Therapist license (or compact privileges if applicable)
- Active CPR/BLS certification (or willingness to obtain upon hire)
- Current professional liability insurance as required by facility
- Post-acute or skilled nursing experience preferred but new grads with strong clinical skills encouraged to apply
- Strong communication skills and ability to work as part of an interdisciplinary team
- Ability to provide care across a seven-day coverage model
Education
- Doctor of Physical Therapy (DPT) or equivalent degree preferred
Knowledge & Skills
- Knowledge of musculoskeletal and neurologic rehabilitation principles
- Skilled in therapeutic exercise, gait training, and functional mobility
- Proficient with documentation systems and rehab outcome measures
- Ability to instruct and train caregivers and family members
- Comfort managing caseloads in a fast-paced post-acute setting
- Good clinical reasoning and goal-directed treatment planning
