Candidate Pay Rate: $69 per hour

Roles & Responsibilities
- Assess patient mobility, strength and functional limitations to build individualized treatment plans
- Develop and implement therapeutic interventions to restore function and independence
- Document progress and outcomes clearly and promptly in the medical record
- Collaborate with nurses, physicians and rehab team members to coordinate patient care
- Educate patients and families on home programs, safety and fall prevention
- Ensure a safe treatment environment and follow infection control and facility policies
- Drive quality improvement by tracking outcomes and participating in team meetings
Qualifications
- Current Physical Therapist license in Massachusetts or ability to obtain before start
- Graduation from an accredited physical therapy program; DPT preferred
- Basic Life Support (BLS) CPR certification
- Experience in skilled nursing, inpatient rehab, or post-acute settings preferred
- Strong documentation and interpersonal skills; teamwork mindset
Education
- Doctor of Physical Therapy (DPT) preferred or graduation from an accredited PT program
Knowledge & Skills
- Skilled in patient assessment and treatment planning
- Proficient with therapeutic exercise, transfer training and gait training
- Familiarity with common rehab modalities and assistive devices
- Comfortable with electronic medical record documentation
- Excellent communication and patient education skills
- Ability to work collaboratively within an interdisciplinary team
