Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment to post-acute and rehab patients
• Develop individualized, goal-directed plans of care and document progress clearly
• Collaborate with nursing, OT, SLP and case management to optimize patient outcomes
• Educate patients and families on safe mobility, home exercise programs, and fall prevention
• Participate in discharge planning to help patients transition safely back to the community
• Maintain accurate clinical records and contribute to quality improvement efforts

Qualifications

• Licensed Physical Therapist in Pennsylvania or eligible to obtain PA licensure prior to start
• Graduate of an accredited physical therapy program (DPT preferred)
• Current BLS (Basic Life Support) certification or willingness to obtain prior to start
• Experience in post-acute, skilled nursing, or rehabilitation settings preferred
• Strong assessment, manual therapy, and therapeutic exercise skills
• Excellent communication and interdisciplinary collaboration skills

Education

• Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy or equivalent considered

Knowledge & Skills

• Skilled in mobility training, gait training and therapeutic exercise prescription
• Knowledge of documentation best practices and electronic medical records
• Ability to perform functional assessments and set measurable rehab goals
• Comfort working with complex medical patients and interdisciplinary teams
• Time management and caseload prioritization to meet patient needs
