Roles & Responsibilities
- Provide skilled evaluation and individualized treatment plans to help patients regain function
- Deliver hands-on therapy, therapeutic exercise, and mobility training with compassion
- Monitor patient progress and adjust interventions to meet rehabilitation goals
- Document treatments and outcomes accurately and timely in the patient record
- Collaborate with nurses, physicians, and the interdisciplinary team to coordinate care
- Educate patients and families on home exercise programs and safe functional strategies
Qualifications
- Active Physical Therapist license in Pennsylvania (or ability to obtain prior to start)
- Current BLS (Basic Life Support) certification or willingness to obtain
- Clinical experience in inpatient rehabilitation or skilled nursing settings preferred
- Strong interpersonal skills and a team-oriented approach to patient care
- Ability to pass credentialing and background checks required by the facility
Education
- Graduate of an accredited Physical Therapy program (DPT preferred)
Knowledge & Skills
- Skilled in patient assessment, therapeutic exercise, and mobility training
- Experience with documentation and electronic health record systems
- Knowledge of safety, fall prevention, and safe patient handling techniques
- Excellent communication skills for patient education and interdisciplinary collaboration
- Ability to prioritize caseload and deliver efficient, evidence-based care
