Roles & Responsibilities

• Provide compassionate direct patient care as a Licensed Practical Nurse, delivering safe and effective treatments
• Administer medications and document responses accurately and promptly
• Collaborate with the interdisciplinary team to develop and update individualized care plans
• Monitor patient status, recognize changes, and escalate concerns to RNs or providers as needed
• Perform skilled nursing procedures and maintain clear, timely charting
• Support patients and families with education, comfort measures, and discharge planning

Qualifications

• Active LPN or RN licensure (LPN is primary for this posting; RN candidates may also apply)
• Valid BLS/CPR certification preferred or willingness to obtain upon hire
• Prior experience in acute care or rehabilitation settings is a plus
• Ability to work overnight/extended shifts and adapt to travel assignment schedules
• Reliable, professional, and a strong team player with excellent communication skills

Education

• Graduate of an accredited practical nursing or registered nursing program

Knowledge & Skills

• Solid clinical assessment and documentation skills
• Medication administration competence and safety focus
• Familiarity with electronic medical records and charting best practices
• Effective communication with patients, families, and multidisciplinary teams
• Ability to prioritize, multitask, and remain calm under pressure
