Candidate Pay Rate: $46 per hour

Roles & Responsibilities
- Provide compassionate direct patient care including medication administration and vital sign monitoring
- Perform timely and accurate documentation in the patient chart and communicate changes to the care team
- Implement individualized care plans and collaborate with nurses, therapists, and physicians
- Assist with activities of daily living and promote patient comfort, dignity, and safety
- Monitor wound care, IVs as appropriate to scope, and report clinical changes promptly
- Support discharge planning and patient education to help residents transition safely
Qualifications
- Valid LPN license in Massachusetts (required)
- Current BLS/CPR certification (required or willing to obtain)
- Previous LPN experience in a rehab, skilled nursing, or long-term care setting preferred
- Reliable, compassionate, and able to work independently as well as in a team
- Ability to pass facility background checks and health screenings
Education
- LPN diploma or certificate from an accredited practical nursing program
Knowledge & Skills
- Strong medication administration and documentation skills
- Skilled in resident assessments and recognizing clinical changes
- Comfortable with wound care, infection control, and rehab nursing practices
- Clear communication and teamwork with interdisciplinary staff and families
- Familiarity with electronic medical records and basic computer skills
