Roles & Responsibilities

• Provide compassionate direct patient care including medication administration and vital sign monitoring
• Perform timely and accurate documentation in the patient chart and communicate changes to the care team
• Implement individualized care plans and collaborate with nurses, therapists, and physicians
• Assist with activities of daily living and promote patient comfort, dignity, and safety
• Monitor wound care, IVs as appropriate to scope, and report clinical changes promptly
• Support discharge planning and patient education to help residents transition safely

Qualifications

• Valid LPN license in Massachusetts (required)
• Current BLS/CPR certification (required or willing to obtain)
• Previous LPN experience in a rehab, skilled nursing, or long-term care setting preferred
• Reliable, compassionate, and able to work independently as well as in a team
• Ability to pass facility background checks and health screenings

Education

• LPN diploma or certificate from an accredited practical nursing program

Knowledge & Skills

• Strong medication administration and documentation skills
• Skilled in resident assessments and recognizing clinical changes
• Comfortable with wound care, infection control, and rehab nursing practices
• Clear communication and teamwork with interdisciplinary staff and families
• Familiarity with electronic medical records and basic computer skills
