Roles & Responsibilities
- Provide compassionate, resident-centered nursing care that supports recovery and dignity
- Administer medications and treatments accurately and safely according to facility policies
- Monitor and document vital signs, changes in condition and response to care in the chart
- Collaborate with the interdisciplinary team to support individualized care plans
- Assist with activities of daily living and support rehabilitation goals for residents
- Maintain a clean, safe environment and follow infection control and safety protocols
Qualifications
- Current LPN license in good standing (state licensure required; bring documentation)
- Graduate of an accredited practical nursing program
- BLS/CPR certification preferred or willing to obtain prior to start
- Experience in long-term care, rehab or skilled nursing settings is a plus
- Current immunizations and ability to pass background check and drug screen
- Reliable, professional, and ready to be a positive member of the care team
Education
- Graduate of an accredited LPN/LVN program; current LPN license required
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Comfortable with charting and electronic health records
- Strong clinical assessment and observation skills
- Good communication and teamwork when coordinating resident care
- Ability to prioritize, manage time and respond quickly in changing situations
- Knowledge of infection control, wound care basics and safety standards
