Roles & Responsibilities

• Provide compassionate, resident-centered nursing care that supports recovery and dignity
• Administer medications and treatments accurately and safely according to facility policies
• Monitor and document vital signs, changes in condition and response to care in the chart
• Collaborate with the interdisciplinary team to support individualized care plans
• Assist with activities of daily living and support rehabilitation goals for residents
• Maintain a clean, safe environment and follow infection control and safety protocols

Qualifications

• Current LPN license in good standing (state licensure required; bring documentation)
• Graduate of an accredited practical nursing program
• BLS/CPR certification preferred or willing to obtain prior to start
• Experience in long-term care, rehab or skilled nursing settings is a plus
• Current immunizations and ability to pass background check and drug screen
• Reliable, professional, and ready to be a positive member of the care team

Education

• Graduate of an accredited LPN/LVN program; current LPN license required

Knowledge & Skills

• Skilled in medication administration and basic nursing procedures
• Comfortable with charting and electronic health records
• Strong clinical assessment and observation skills
• Good communication and teamwork when coordinating resident care
• Ability to prioritize, manage time and respond quickly in changing situations
• Knowledge of infection control, wound care basics and safety standards
