Roles & Responsibilities

• Provide attentive, compassionate nursing care to residents according to plan of care
• Administer medications and treatments accurately and document thoroughly
• Monitor resident conditions, report changes promptly, and escalate concerns to the care team
• Support rehabilitation goals by assisting with daily living activities and therapeutic tasks
• Collaborate with nurses, therapists, and physicians to deliver coordinated care
• Maintain a safe, clean environment and follow infection control and facility policies
• Build meaningful relationships with residents and families to promote dignity and comfort

Qualifications

• Current Licensed Practical Nurse (LPN) license — must be active and in good standing
• Basic Life Support (BLS) certification required; additional certifications a plus
• Previous experience in long-term care, rehabilitation, or skilled nursing preferred
• Strong clinical assessment, medication administration, and documentation skills
• Excellent communication, teamwork, and a resident-centered attitude

Education

• LPN diploma or certificate from an accredited program
• High school diploma or equivalent

Knowledge & Skills

• Familiarity with long-term care and rehab nursing practices
• Ability to perform wound care, medication administration, and basic nursing procedures
• Skilled in documenting electronic medical records and care plans
• Good time management, prioritization, and ability to work in a fast-paced setting
• Professional bedside manner and strong interpersonal skills
