Roles & Responsibilities

• Provide compassionate, hands-on personal care to residents, promoting dignity and comfort
• Assist with activities of daily living including bathing, dressing, toileting, and mobility
• Monitor and report changes in patient condition promptly to the nursing team
• Accurately document care provided and vital signs in the patient record
• Support safe transfers and fall prevention practices to protect residents
• Collaborate with nurses and interdisciplinary staff to deliver individualized care plans

Qualifications

• Valid LNA (Licensed Nursing Assistant) certification required; ability to work in NH (and VT if applicable) preferred
• Current basic life support (BLS/CPR) certification preferred or willing to obtain
• Comfortable working overnight shifts and performing hands-on resident care
• Reliable, professional, and able to pass background and health screenings
• Strong communication skills and a team-oriented attitude

Education

• High school diploma or equivalent and completed LNA training program

Knowledge & Skills

• Solid knowledge of ADLs and basic patient care techniques in long-term care
• Ability to take and record vital signs accurately
• Familiarity with safe patient handling and infection control practices
• Basic charting and documentation skills
• Compassionate bedside manner and strong observational skills
