Roles & Responsibilities

• Provide compassionate personal care and assist patients with activities of daily living including bathing, dressing and toileting
• Take and record vital signs accurately and report changes promptly to nursing staff
• Assist with safe patient transfers, ambulation and positioning
• Maintain a clean, safe and comfortable environment for patients
• Document care and observations clearly and timely to support care continuity
• Support patients emotionally and promote dignity and independence
• Collaborate with nurses and the care team to deliver high-quality patient-centered care

Qualifications

• Current Licensed Nursing Assistant (LNA) certification (valid in New Hampshire; VT credentials a plus)
• CPR/BLS certification preferred or willingness to obtain
• Previous experience in long-term care or similar settings preferred
• Ability to pass background check and drug screening
• Strong communication, teamwork and patient-focused attitude

Education

• High school diploma or GED
• Completion of a state-approved nurse aide program

Knowledge & Skills

• Fundamentals of personal care and infection control
• Basic vital signs measurement and documentation
• Safe patient handling and transfer techniques
• Effective communication with patients, families and care teams
• Time management and the ability to prioritize patient needs
