Roles & Responsibilities

• Deliver compassionate, hands-on nursing care to residents in a long-term/acute care setting
• Administer medications accurately and document all care in the electronic chart
• Monitor and record vital signs, identify changes, and promptly report concerns
• Collaborate with RNs, physicians, and the care team to create and follow individualized care plans
• Provide patient and family education to promote recovery, safety, and comfort
• Assist with admissions, transfers, and discharges to ensure smooth transitions
• Maintain accurate, timely documentation and uphold facility policies and safety standards

Qualifications

• Current Licensed Practical Nurse (LPN) license valid for practice in New Hampshire
• Basic Life Support (BLS) certification from an accredited provider (AHA preferred)
• Current immunizations and compliance with facility-required health screenings
• Clear background check and ability to meet facility onboarding requirements
• Previous clinical experience is welcome; experience in long-term care or med-surg is a plus

Education

• Completion of an accredited Practical Nursing program (LPN diploma or certificate)
• High school diploma or equivalent

Knowledge & Skills

• Strong clinical assessment and patient observation skills
• Medication administration and safe documentation practices
• Basic wound care and infection prevention knowledge
• Comfortable using electronic medical records and standard nursing documentation
• Excellent communication, teamwork, and patient-centered care approach
• Ability to prioritize tasks and stay organized during a busy shift
