Roles & Responsibilities

• Provide compassionate, patient-centered nursing care as an LPN
• Administer medications and treatments accurately and safely
• Monitor and record patient vital signs and report changes promptly
• Collaborate with RNs, providers, and the care team to support patient plans
• Document patient care thoroughly in the electronic medical record
• Support patient comfort, education, and discharge planning efforts
• Contribute to a positive, team-focused environment and uphold facility standards

Qualifications

• Current Licensed Practical Nurse (LPN) license — eligible for or licensed in New Hampshire
• Clinical experience as an LPN caring for adult or geriatric patients preferred
• Current BLS/CPR certification or willingness to obtain
• Excellent communication and interpersonal skills
• Ability to handle documentation and medication administration with attention to detail

Education

• Diploma or certificate in Practical Nursing (LPN/LVN) or equivalent

Knowledge & Skills

• Strong fundamentals in medication administration and basic nursing procedures
• Skilled at taking and interpreting vital signs and patient assessments
• Comfortable with electronic medical records and accurate charting
• Good clinical judgement and the ability to escalate concerns appropriately
• Team-oriented mindset and patient-first approach
