Roles & Responsibilities
- Provide compassionate, skilled daily nursing care to residents in a skilled nursing/rehab setting
- Administer medications, document treatments, and monitor patient responses accurately
- Collaborate with RNs, therapy staff, and physicians to implement individualized care plans
- Perform wound care, catheter care, and other assigned clinical procedures per facility policy
- Educate residents and families about care plans, medications, and discharge goals
- Report changes in condition promptly and participate in interdisciplinary team meetings
- Support a positive, safe environment that promotes resident dignity and recovery
Qualifications
- Current and unrestricted LPN license (Licensed Practical Nurse) — Massachusetts licensure required
- Recent experience in skilled nursing, long-term care, or rehabilitation preferred
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Comfortable with medication administration, wound care, and documentation systems
- Strong communication skills and ability to work collaboratively in a team
Education
- Completion of an accredited LPN/LVN program
- High school diploma or GED
Knowledge & Skills
- Solid clinical assessment and vital-sign monitoring skills
- Medication administration and accurate charting
- Knowledge of infection control and patient safety practices
- Ability to prioritize tasks in a fast-paced rehab/nursing environment
- Empathy, patience, and strong resident-centered care approach
