Roles & Responsibilities

• Provide compassionate, skilled daily nursing care to residents in a skilled nursing/rehab setting
• Administer medications, document treatments, and monitor patient responses accurately
• Collaborate with RNs, therapy staff, and physicians to implement individualized care plans
• Perform wound care, catheter care, and other assigned clinical procedures per facility policy
• Educate residents and families about care plans, medications, and discharge goals
• Report changes in condition promptly and participate in interdisciplinary team meetings
• Support a positive, safe environment that promotes resident dignity and recovery

Qualifications

• Current and unrestricted LPN license (Licensed Practical Nurse) — Massachusetts licensure required
• Recent experience in skilled nursing, long-term care, or rehabilitation preferred
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable with medication administration, wound care, and documentation systems
• Strong communication skills and ability to work collaboratively in a team

Education

• Completion of an accredited LPN/LVN program
• High school diploma or GED

Knowledge & Skills

• Solid clinical assessment and vital-sign monitoring skills
• Medication administration and accurate charting
• Knowledge of infection control and patient safety practices
• Ability to prioritize tasks in a fast-paced rehab/nursing environment
• Empathy, patience, and strong resident-centered care approach
