Roles & Responsibilities

• Provide compassionate patient care following the plan of care and facility standards
• Administer medications and treatments accurately and document thoroughly
• Monitor patient condition, report changes promptly, and escalate when needed
• Collaborate with nurses, therapists, and interdisciplinary team members to optimize outcomes
• Support patient and family education with clarity and empathy
• Maintain accurate clinical documentation and participate in unit-level quality initiatives

Qualifications

• Current Licensed Practical Nurse (LPN) license for the state of Pennsylvania (or compact/reciprocal as applicable)
• Current Basic Life Support (BLS) certification preferred or willingness to obtain
• Prior LPN clinical experience preferred; new graduates from LPN programs are welcome to apply
• Ability to pass background checks and drug screening as required by the facility
• Strong communication skills and a patient-focused mindset

Education

• Completion of an accredited LPN/LVN program or diploma

Knowledge & Skills

• Solid knowledge of medication administration and nursing fundamentals
• Comfortable with charting, electronic health records, and clinical documentation
• Ability to prioritize tasks in a fast-paced long-term care or skilled nursing setting
• Team-oriented approach and strong interpersonal skills
• Understanding of infection control practices and patient safety principles
