Roles & Responsibilities
- Deliver compassionate, skilled nursing care to residents at the point of need
- Administer medications and treatments safely and document thoroughly
- Perform focused patient assessments and communicate changes to the care team
- Support residents with activities of daily living while promoting independence
- Maintain accurate electronic medical records and complete required documentation
- Collaborate with nurses, therapists, and support staff to create meaningful care plans
- Adhere to infection control practices and facility policies to keep patients safe
Qualifications
- Active Licensed Practical Nurse (LPN) license in Massachusetts - required
- Current BLS/CPR certification preferred or willingness to obtain promptly
- Previous LPN experience in a skilled nursing, rehab, or long-term care setting preferred
- Comfortable with medication administration, wound care basics, and resident monitoring
- Strong communication skills and a team-oriented attitude
Education
- Completion of an accredited Practical Nursing program or equivalent
Knowledge & Skills
- Medication administration and accurate MAR documentation
- Basic wound care and dressing changes
- Patient assessment and vital sign interpretation
- Electronic medical record (EMR) familiarity
- Time management and prioritization in a fast-paced setting
- Compassionate resident-centered care and effective teamwork
