Roles & Responsibilities

• Administer medications and treatments safely and accurately according to orders
• Perform regular patient assessments and document findings clearly in the chart
• Collaborate with nurses, therapists and physicians to deliver coordinated care
• Provide compassionate bedside care and patient education to support recovery
• Monitor patient status and escalate changes promptly to the care team
• Maintain infection control standards and safety protocols at all times

Qualifications

• Active Licensed Practical Nurse (LPN) license — must be valid in Pennsylvania
• Current BLS/CPR certification required
• Ability to pass background check, drug screen and health requirements
• Prior clinical experience preferred (long-term care, rehab or acute care experience helpful)
• Strong communication and teamwork skills; professionalism and reliability

Education

• Graduate of an accredited practical nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and vital signs monitoring
• Familiarity with electronic health records and charting
• Infection prevention and standard precautions
• Time management and prioritization in a clinical setting
• Patient-focused communication and family education
