Roles & Responsibilities

• Assess and monitor patient condition, documenting changes clearly and promptly
• Administer medications and treatments according to plan and facility policy
• Collaborate with RNs and interdisciplinary team to deliver coordinated care
• Provide compassionate patient education and support to residents and families
• Perform wound care, basic procedures, and assist with activities of daily living
• Maintain accurate, timely charting and assist in care planning

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current Basic Life Support (BLS) certification or ability to obtain prior to start
• Valid ID, ability to pass background and drug screening as required by facility
• Professional, patient-centered approach and good communication skills

Education

• Graduate of an accredited practical nursing program or equivalent credential

Knowledge & Skills

• Strong medication administration and documentation practices
• Patient assessment and vital signs monitoring
• Familiarity with care planning and interdisciplinary communication
• Time management and prioritization in a fast-paced clinical setting
• Comfortable using electronic health records and basic clinical tech
