Roles & Responsibilities

• Provide safe, compassionate nursing care to residents, following facility policies and care plans
• Perform thorough patient assessments and document changes clearly and promptly
• Administer medications and treatments accurately and on schedule
• Collaborate with interdisciplinary team members to coordinate patient-centered care
• Educate residents and families about care plans and health promotion
• Respond effectively to emergencies and escalate concerns when needed
• Maintain accurate electronic health record documentation and handoffs

Qualifications

• Active LPN license (or RN license if applying as RN) — Delaware licensure preferred
• Current BLS/CPR certification required
• Up-to-date immunizations and TB screening per facility policy
• Previous experience in long-term care, skilled nursing, or similar settings preferred
• Strong communication skills, ability to work independently and as part of a team
• Willingness to work the scheduled 2:45 PM - 11:15 PM shift

Education

• Licensed Practical Nurse: Graduate of an accredited Practical Nursing program
• Registered Nurse (if applying as RN): ADN or BSN from an accredited nursing program

Knowledge & Skills

• Medication administration and documentation best practices
• Wound care, ADL assistance, and basic restorative nursing skills
• Familiarity with electronic health record systems
• Patient assessment and clinical observation skills
• Time management and prioritization in a busy care environment
• Professional bedside manner and effective family communication
