Shift Type: Rotating 7 days on / 7 days off, Evening/Night 4:00 PM - 2:30 AM (last two nights of the rotation extend to 3:30 AM to meet 72 hours total). Rotation starts 10/1/2025 - 10/7/2025 and continues every other week. Every-other-weekend (EOW) coverage required; pharmacists work any weekend or holiday that falls within their scheduled 7-day rotation.

Roles & Responsibilities

• Verify and safely dispense medications using EPIC EMR, ensuring accuracy for every patient
• Provide clinical pharmacy support during evening and overnight hours to the care team
• Collaborate with physicians, nurses, and other clinicians to optimize medication therapy
• Perform medication reconciliation and maintain accurate medication profiles
• Support emergency responses and clinical decision-making during night shift operations
• Maintain compliance with state licensing, hospital policies, and credentialing requirements
• Complete required Trinity skills checklists and participate in mandatory training and testing

Qualifications

• Active Indiana State Pharmacist License required
• BLS certification required
• Trinity Skills Checklist completion and License/Certificate Verification required
• Preferred EPIC EMR experience
• Willingness and ability to work 7 days on / 7 days off rotation, including nights, weekends, and holidays as scheduled
• Must meet personal health and screening requirements (fit test, TB screening, immunizations, 10-panel drug screen, etc.)

Education

• Doctor of Pharmacy (PharmD) or equivalent preferred

Knowledge & Skills

• Strong medication safety and clinical pharmacy knowledge
• Proficiency with EPIC EMR workflows preferred
• Excellent communication and teamwork skills for multidisciplinary collaboration
• Attention to detail and strong accuracy in dispensing and documentation
• Ability to work independently during overnight shifts and handle urgent clinical scenarios
