Roles & Responsibilities

• Perform thorough diagnostic assessments to identify patient needs and craft individualized treatment plans
• Implement habilitation and rehabilitation programs that help patients regain function and independence
• Deliver skilled outpatient physical therapy services while also supporting SNF/inpatient cases as needed
• Collaborate with interdisciplinary care teams to coordinate holistic patient care
• Document evaluations, progress notes, and treatment plans accurately in Meditech/EMR
• Provide patient and family education to maximize recovery and promote long-term outcomes
• Adapt care and schedules based on hospital operations, including weekends and holidays

Qualifications

• Active Hawaii State License required by start of assignment (travelers must apply upon acceptance if not currently licensed)
• BLS (Basic Life Support) certification required
• Minimum 6 months of work experience in at least one adult setting: Acute, SNF, rehab hospital, or outpatient
• Experience delivering outpatient physical therapy services; comfort treating a wide range of adult cases
• Comfortable using Meditech or similar EMR systems

Knowledge & Skills

• Skilled in diagnostic assessment, treatment planning, and progress monitoring
• Strong knowledge of habilitation and rehabilitation principles for adult patients
• Ability to manage caseloads across outpatient and occasional inpatient/SNF settings
• Proficient documentation practices and familiarity with Meditech EMR
• Excellent communication and team collaboration skills
• Flexible mindset to cover variable schedules, weekends, and holidays as needed
