Roles & Responsibilities
- Provide compassionate direct patient care and support daily living needs
- Administer medications and treatments accurately and safely
- Monitor and record patient vital signs and clinical changes, escalating concerns when needed
- Document care in the clinical record clearly and timely to support continuity
- Collaborate with RNs, providers, and the care team to implement care plans
- Educate patients and families on care, medications, and follow-up instructions
- Maintain a safe, clean environment and follow facility policies and infection control practices
Qualifications
- Active Licensed Practical Nurse (LPN) license — eligible to practice in Maine preferred
- Current CPR/BLS certification (or willing to obtain upon hire)
- Comfortable with medication administration and basic clinical assessments
- Strong communication skills and a team-first attitude
- Ability to pass required background checks and meet facility onboarding requirements
Education
- Graduate of an accredited Practical Nursing program (LPN diploma or equivalent)
Knowledge & Skills
- Clinical assessment and basic patient monitoring
- Medication administration and documentation best practices
- Familiarity with electronic medical/health records or willingness to learn
- Effective communication with patients, families, and interdisciplinary teams
- Time management and the ability to prioritize tasks in a busy setting
