Roles & Responsibilities
- Provide compassionate, hands-on patient care that supports recovery and dignity
- Administer medications and treatments safely and accurately according to policy
- Monitor and document vital signs and patient status to keep the care team informed
- Collaborate with RNs, therapists, and interdisciplinary staff to deliver coordinated care
- Educate patients and families on care plans, medications, and discharge instructions
- Maintain clear, timely electronic documentation and handoff communication
- Support infection control, safety protocols, and a positive patient experience
Qualifications
- Current Licensed Practical Nurse (LPN) license or eligible for licensure in Maine
- Basic Life Support (BLS) / CPR certification (or willingness to obtain)
- Prior experience in long-term care, skilled nursing, or similar clinical setting preferred
- Ability to pass background checks, required health screenings, and meet facility onboarding needs
- Strong communication skills and comfort working as part of a multidisciplinary team
Education
- Graduate of an accredited practical nursing program (LPN diploma or certificate)
Knowledge & Skills
- Medication administration and documentation best practices
- Patient assessment fundamentals and monitoring for change in condition
- Familiarity with electronic medical records and accurate charting
- Basic wound care and preventive nursing interventions
- Time management, prioritization, and teamwork in a fast-paced setting
- Compassionate patient- and family-centered communication
