Roles & Responsibilities

• Provide compassionate, hands-on patient care that supports recovery and dignity
• Administer medications and treatments safely and accurately according to policy
• Monitor and document vital signs and patient status to keep the care team informed
• Collaborate with RNs, therapists, and interdisciplinary staff to deliver coordinated care
• Educate patients and families on care plans, medications, and discharge instructions
• Maintain clear, timely electronic documentation and handoff communication
• Support infection control, safety protocols, and a positive patient experience

Qualifications

• Current Licensed Practical Nurse (LPN) license or eligible for licensure in Maine
• Basic Life Support (BLS) / CPR certification (or willingness to obtain)
• Prior experience in long-term care, skilled nursing, or similar clinical setting preferred
• Ability to pass background checks, required health screenings, and meet facility onboarding needs
• Strong communication skills and comfort working as part of a multidisciplinary team

Education

• Graduate of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Patient assessment fundamentals and monitoring for change in condition
• Familiarity with electronic medical records and accurate charting
• Basic wound care and preventive nursing interventions
• Time management, prioritization, and teamwork in a fast-paced setting
• Compassionate patient- and family-centered communication
