Roles & Responsibilities
- Provide compassionate, patient-centered care to assigned residents
- Administer medications and treatments safely and accurately
- Monitor and document patient condition and response to care
- Collaborate with the interdisciplinary team to create and follow care plans
- Assist with admissions, transfers and discharges as needed
- Communicate effectively with patients, families and clinical staff
Qualifications
- Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) credential
- Active license in accordance with facility and state requirements
- Clinical experience in a long-term care or rehabilitation setting preferred
- Current Basic Life Support (BLS) certification or willingness to obtain
- Strong communication and teamwork skills
Education
- Completion of an accredited LPN or RN nursing program (education appropriate to licensure)
Knowledge & Skills
- Medication administration and documentation best practices
- Patient assessment and basic clinical interventions
- Familiarity with care plan development and nursing process
- Ability to work in a fast-paced clinical environment with empathy
- Basic computer skills for electronic charting and documentation
