Roles & Responsibilities

• Provide compassionate hands-on care and assist residents with activities of daily living (bathing, dressing, toileting, feeding)
• Monitor and report changes in patient condition to the nursing team promptly
• Support safe transfers and mobility, using proper body mechanics and assistive devices
• Document care accurately and timely in the chart, keeping patient records up to date
• Maintain a clean, safe environment and follow infection control policies
• Collaborate with nurses, therapists, and interdisciplinary team members to deliver person-centered care
• Offer emotional support and build trusting relationships with residents and families

Qualifications

• Certified Nursing Assistant (CNA) certification — required
• Active/state-appropriate CNA credentials or ability to obtain state authorization
• Current CPR/BLS certification preferred or willing to obtain
• Previous experience in long-term care or skilled nursing preferred but new grads with strong clinical skills are welcome
• Strong communication skills, reliability, and a team-first attitude

Education

• High school diploma or GED
• Completion of an accredited CNA training program

Knowledge & Skills

• Fundamental patient care and safety practices
• Basic vital signs monitoring and reporting
• Infection prevention and proper PPE use
• Clear charting and time management
• Compassionate interpersonal skills and conflict resolution
• Ability to follow care plans and prioritize resident needs
