Shift Type: Monday-Friday, 8:00 AM - 4:30 PM Day; weekend coverage every other weekend (variable) — Day shift (may float to cover ICU, Med-Surg, Telemetry, Observation and ED as needed)
Hours per Week: 40
Candidate Pay Rate: $67 per hour

Roles & Responsibilities
- Coordinate and manage inpatient case loads with compassion to ensure safe, timely discharges
- Lead discharge planning efforts: arrange DME, home health, hospice, and post-acute placements
- Collaborate with interdisciplinary teams (therapy, social work, pharmacy, radiology, etc.) to remove barriers to care
- Document thoroughly and accurately in Epic to keep patient records clear and current
- Obtain authorizations and work with payors on medical necessity, continued stay reviews and appeals as needed
- Perform weekend-focused assessments and discharges, prioritizing patient safety and continuity of care
- Support and mentor Case Management Assistants by delegating tasks and coordinating workflows
- Float across units (ICU, Med-Surg, Telemetry, Observation, ED) to meet hospital needs and maintain quality
Qualifications
- Active RN license (Oregon) or ability to obtain — pending licensure considered (PEND LIC noted)
- Minimum 1 year of RN experience; 1 year of case management experience preferred
- Charting experience in Epic is required
- BLS certification preferred
- Comfortable floating to multiple acute care settings and working weekends as scheduled
- Strong communication and care coordination skills with a patient-first mindset
- Travel experience NOT required
Knowledge & Skills
- Case management fundamentals: care coordination, discharge planning, patient advocacy
- Utilization review concepts: continued stay reviews, appeals and denials, medical necessity
- Familiarity with regulatory frameworks: CMS, DRG, HIPAA, NCQA, OSHA, The Joint Commission
- Proficiency with Epic charting and documentation workflows
- Ability to navigate post-acute placement options: SNF, home health, hospice, assisted living
- Interdisciplinary communication and collaboration across clinical teams
- Authorization and insurance communication skills to secure necessary services
