Roles & Responsibilities

• Perform thorough diagnostic assessments and craft individualized treatment plans to help adults regain function and independence.
• Deliver skilled physical therapy services across SNF, inpatient, outpatient, and rehab hospital settings as needed.
• Document care accurately and promptly in Meditech EMR to ensure continuity and quality of care.
• Collaborate with interdisciplinary teams to coordinate patient-centered rehabilitation programs.
• Educate patients, families, and caregivers on home programs and safety to support long-term outcomes.
• Be available to cover shifts, weekends, and holidays based on operational needs and unit coverage.

Qualifications

• Active Hawaii Physical Therapy license required by start of assignment. Candidates not currently licensed in HI must apply upon acceptance.
• BLS certification required.
• Minimum 6 months of work experience in at least one adult setting (Acute, SNF, Rehab Hospital, or Outpatient).
• Comfort working with adult populations across SNF/inpatient and outpatient environments.
• Familiarity with Meditech EMR or willingness to learn quickly.
• Ability to work flexible schedules including weekends and holidays.

Knowledge & Skills

• Patient assessment and treatment planning for adult rehabilitation populations
• Experience in SNF, inpatient, outpatient, or rehab hospital settings
• Skilled documentation and charting in Meditech EMR
• Clinical decision making under general supervision
• Strong communication and teamwork across interdisciplinary teams
• Ability to manage caseloads and prioritize patient needs
