Roles & Responsibilities

• Assess patients in the home setting and establish individualized physical therapy plans of care
• Deliver direct patient care using evidence-based interventions and appropriate modalities
• Train patients and caregivers in safe ADLs, transfers, home exercise programs, and use of rehabilitation equipment
• Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care
• Complete thorough, timely documentation and OASIS assessments to support care plans and outcomes
• Recommend, fit, and instruct in use of assistive devices, splints, orthoses, prostheses and wheelchairs
• Monitor patient progress, adjust treatment plans, and communicate changes to the care team
• Work independently in patients' homes while maintaining high standards of safety and professionalism

Qualifications

• Home Health Certification (required)
• Active American Heart Association BLS (AHA) (required)
• Massachusetts RN license uploaded to profile (required)
• Valid driver license (required) and ability to travel to patient homes
• Minimum 2+ years combined acute nursing care (hospital-based) and home care experience (required)
• Proficiency with OASIS documentation (required, non-negotiable)
• Ability to work independently with minimal supervision
• Acute care/hospital-based experience (preferred)
• Supervisor-signed reference from within the past two years (required for submission)

Knowledge & Skills

• Strong knowledge of physical therapy principles and home health best practices
• Proficient with OASIS assessments and home health documentation
• Experience with rehabilitation equipment: splints, orthoses, prostheses and wheelchairs
• Skilled in ADL assessment and training
• Comfortable using therapeutic modalities appropriate to the patient population
• Excellent clinical decision-making and patient education skills
• Good communication and collaboration with interdisciplinary teams
